Social Worker (East, Office Hours, Senior Daycare) #HRY

PrimaryResponsibilities and Duties (90%)

• Provide psycho-social support for patients and their families through case management, counselling, financial assessment and recommend resources where necessary.
• Participate in the development of the care plan and assist staff members in understanding the significant social and emotional factors related to clients health problems.
• Collaborate with other healthcare professionals and family members/caregivers to determine appropriate care plan.
• Experiences in Medifund will be an advantage.

SecondaryResponsibilities and duties (10%)

• Any other ad hoc projects as assigned by supervisor.
