Job Description

Responsibilities

  • Conduct case assessment and formulate Individualized Care Plan for Persons with Dementia and their carers.
  • Provide intervention such as psychoeducation, supportive counselling and service linkages.
  • Liaise and collaborate with multidisciplinary professionals and external service providers for holistic care management.
  • Provide case recordings and relevant documentations.
  • Participate in team meetings and case conferences as required.
  • Assist in the preparation of data and statistics for programme evaluation and reporting purposes.
  • Plan and facilitate Caregiver Support Group sessions.
  • Plan, support and facilitate outreach events and talks.
  • Any ad-hoc tasks as and when required.

Requirements

  • Degree or post-graduate diploma in social work, psychology or counselling. Relevant degree in allied health profession will be considered.
  • Preferably 2 years of healthcare/community care clinical experience.
  • Good oral, written and interpersonal communications skills.
  • Able to work on occasional Saturdays and weekday evenings.
  • Prior working experience in dementia care will be an added advantage.
