Social Worker (Community Connection)
Job Scope:
Case Management, Intervention & Evaluation
- Assess referral cases to ensure suitability for intake.
- Conduct needs and intake assessments (identify severity, key risks, and protective factors).
- Use different modes of engagement for assessment, intervention, and monitoring.
- Develop and implement intervention plans (analyse, evaluate, and formulate strategies).
- Engage beneficiaries and families to sustain problem-solving efforts.
- Conduct case reviews, follow up on plans, and recommend modifications where necessary.
- Document and maintain case files according to required standards.
- Assist in group work for target beneficiaries and families.
- Work closely with research team to evaluate effectiveness of programmes and develop strategies to increase outreach and engagement.
- Assist in data collection and support action research activities.
Professional Development
. Participate in developmental programmes and supervision for personal and professional growth.
Programme, Event Management & Stakeholder Engagement
- Manage and execute organisation's annual signature events and intervention programmes in accordance with organisational objectives and standards.
- Plan and coordinate logistics, timelines, budget, and manpower resources for events and programmes.
- Establish meaningful collaboration with corporate and institutional partners, as well as volunteers/voluntary groups for programme delivery.
- Network with other agencies and tap on community resources to provide intervention support.
- Assist in the promotion of programmes and advise beneficiaries and families on relevant services.
- Update database for statistical reporting and prepare post-event reports.
- Perform job-specific projects and duties assigned by supervisor.
Requirements
- At least Bachelor of Social Work or its equivalent
- At least 1-2 years of direct practice experience
- Preferably a Registered Social Worker (RSW) with the Singapore Association of Social Workers (SASW)
- Casework knowledge and ability to engage, assess, provide intervention and evaluate practice
- Committed to social vision
- Ability to build and sustain collaborative working relationships
- Cooperate with team members / key social workers to deliver social work outcomes

You should have:
- Strong programme management skills, aligning initiatives with organisational mission and sector priorities, with ability to develop basic Theory of Change and logic models.
- Able to identify ground issues and recommend strategic programme improvements.
- Competent in administering evaluation tools and collecting participation and feedback data.
- Sound casework capabilities, including engagement, assessment, intervention, and evaluation of beneficiaries and families.
- Able to apply relevant social work theories, develop care plans, and demonstrate reflective practice (e.g., genograms, social reports).
- Basic group facilitation skills and ability to co-facilitate programmes.
- Good understanding of local community systems, policies, and available resources.
- Able to empower beneficiaries and families to access appropriate support services.
- Committed to social service values, collaborative teamwork, and delivering positive social work outcomes.
