Your Role

As a Social Worker/Case Manager, you will play a key role in supporting residents through comprehensive case management and care coordination. You will work closely with residents, multidisciplinary teams, families, and community partners to promote residents well-being and independence.

Key Responsibilities

  • Conduct comprehensive assessments of residents physical, psychological, social, and financial needs.
  • Develop, implement, and regularly review Individual Care Plans (ICP) to support residents rehabilitation and well-being.
  • Coordinate care with healthcare professionals, therapists, social workers, and community agencies to ensure holistic support.
  • Provide supportive counselling to residents and deliver psychoeducation to enhance their understanding of mental health conditions, treatment, coping strategies, and recovery.
  • Advocate for residents rights and facilitate effective communication between residents, families, and the multidisciplinary team.
  • Plan, organise, and evaluate programmes that promote residents rehabilitation, social integration, and quality of life.
  • Respond appropriately to crisis situations and provide timely intervention when required.
  • Maintain accurate case documentation and ensure compliance with organisational policies and regulatory requirements.

What We're Looking For

  • Diploma or Degree in Social Work, Psychology, Counselling, Nursing, or a related discipline.
  • Relevant experience in case management, healthcare, social services, or community care is preferred.
  • Strong interpersonal, communication, and problem-solving skills.
  • Compassionate, organised, and able to work effectively within a multidisciplinary team.
  • Proficient in documentation and basic computer applications.
