Social Work/ Social Service Associate (CREST - West Region)

The Community Outreach Team (CREST) serves as a mental health first-stop touchpoint. CREST promotes early identification of at-risk individuals and provides psychosocial intervention to support those with mental health needs and their caregivers in the community.

. Promote early recognition of signs and symptoms of mental health conditions and/or dementia.

. Improve mental health conditions and/or dementia literacy by organising targeted outreach events and activities to provide mental health and/or dementia information and education to residents and caregivers in the region.

. Provide psychosocial intervention such as techniques to reduce the risk of escalation of mental health needs.

. Provide collaborative and coordinated care for clients and caregivers through a case management approach.

. Work in partnership with health and community care partners and key stakeholders to co-manage and support persons with mental health conditions and dementia in the community.

Responsibilities
The Social Work / Social Service Associate is responsible for supporting initial triage and screening, following-up with intervention plan as per the prescribed care plan under the guidance of the Supervisor, and conducting the engagement and preventive activities to the clients and/or caregivers in an individual or group-based setting. You will assist in coordinating casework, groupwork and community development activities and programmes, supporting community events, performing administrative functions and planning and implementation of targeted screening and cognitive stimulation activities.

Programme Implementation

. Work closely with key stakeholders to implement programmes and activities for residents in the West (Jurong West, Pioneer, Nanyang, Ayer-Rajah & West Coast) and Central (Potong Pasir & Braddell Heights) regions.

. Assist in running of targeted outreach and cognitive stimulation activities.

. Support in initial assessment of walk-ins and referral cases, follow up with intervention plans for clients as per the prescribed care plan and under guidance from the Supervisors.

. Support in the monitoring the clients conditions and progress and escalate to the Supervisors where necessary.

Ancillary Duties

. Support the operations of Viriya Mental Wellness Hub located at Kent Road.

. Provide logistical or operational support for clients and/or caregivers.

. Planning and executing events within the organisation.

. Any other duties, as assigned.
