Social Work Associate, Senior Service

RESPONSIBILITIES

As a Social Work Associate within the Senior Services team, you will work to promote social-health wellbeing of seniors by engaging them meaningfully.

Role & Responsibilities:

• Handle case related enquiry from clients and the community.
• Conduct regular home visits to monitor the well-being of clients.
• Assist social workers in casework assessment and interventions.
• Collaborate with external healthcare partners to coordinate seamless care.
• Maintain accurate and organized records and files.
• Handle administrative tasks, including data collection and report preparation.
• Participate in internal and external training, supervision and team meetings.
• Carry out any other duties as assigned by HOD and supervisor.

QUALIFICATIONS

• Preferred Polytechnic Diploma in Social Work other diplomas can be considered.
• Passionate to work with seniors and their caregivers in community setting.
• Ability to work independently and as part of a team.
• Strong communication and organizational skills.
• Effective written and oral communication in English.
• Fluency in Mother Tongue language and/or dialects will be advantageous.
• Prior experience in social service &/or healthcare setting preferred.
