Job Description

Brief Summary

Join a dynamic team dedicated to enhancing caregiver support through outreach and engagement initiatives. Your role will involve connecting caregivers with essential services and facilitating meaningful interactions within the Care Guides Programme.

Responsibilities

  • Drive outreach and engagement initiatives to promote access to the Care Guides Programme and related services. Conduct initial contact and perform basic needs assessments for caregivers and potential Care Guides. Manage the matching and onboarding processes between Care Guides and caregivers, ensuring a seamless experience. Provide ongoing support for caregivers and Care Guides, including regular check-ins and follow-up communications. Handle administrative tasks to support programme operations, including scheduling, data entry, and record maintenance. Oversee programme touchpoints on digital platforms and respond to inquiries to enhance user engagement. Support data tracking and reporting related to programme activities and outcomes. Assist in organizing training sessions, meetings, and engagement activities, including logistics and related support. Collaborate closely with the interdisciplinary team to ensure effective programme delivery and care coordination. Participate in supervision, training, and team meetings aimed at professional development. Undertake any additional duties as assigned by the reporting manager.

Requirements

  • Minimum Diploma or Degree in Social Work, Psychology, Social Sciences, or a related field. Relevant experience of 1-3 years in social service, community work, or healthcare settings is preferred. Strong interpersonal and communication skills to effectively engage with caregivers and volunteers. Highly organized and detail-oriented with proven ability to manage administrative and coordination tasks effectively. Genuine passion for caregiver support and community-based services.
