Case Support & Management:
. Support Social Workers/ Case Managers in conducting assessments, maintaining case documentation and following up on care plans.
. To accompany patients for external appointments (eg. Bank or HDB visits etc).
Administrative Support:
. Ensure timely and accurate updates of case notes and proficiency in MS Excel.
Volunteer Management:
. Plan and conduct volunteer activities for patients.
. Coordinate with relevant teams to support implementation & execution.
Stakeholder Liaison:
. Collaborate with relevant agencies and families to coordinate care plans.
. To participate in multidisciplinary team meetings.
Qualification:
. Diploma in Social Work or related field (Psychology/Sociology)
Requirements:
. Experience in community or healthcare services would be an added advantage.
. Passionate about community services and a collaborative team player.
. Strong communication and interpersonal skills.
. Proficient in Microsoft Office applications, especially in the Excel application.
. Adaptable to changes, and a proactive attitude towards learning and service improvement.
. Good time management and organisational skills.
. Multi-tasker with a strong work ethic.
