Role Description
This is a full-time, on-site role for a Social Work Associate based in Singapore. The Social Work Associate will handle responsibilities such as providing social work and medical social work services, managing cases to support residents in addressing their needs, and collaborating with other professionals to coordinate care and services. The role involves communication with residents, their families, and relevant stakeholders to ensure tailored, effective support is delivered.
Qualifications
- Proficiency in Social Work and Medical Social Work practices
- Strong skills in Case Management and Social Services to provide comprehensive support
- Excellent Communication skills for interactions with residents, families, and stakeholders
- Ability to work collaboratively in a multidisciplinary team
- Relevant diploma in Social Work
- Experience working in eldercare or healthcare environments is an advantage
- Empathy, patience, and strong interpersonal skills
