To accompany residents to banks (for deposit/withdrawal of money, replacement of bank book, etc.), Housing & Development Board (for returning of keys, retrieving of personal belongings from rental units, etc.) and as determined by the case.
To maintain records to facilitate submission of the periodic returns and ad hoc reports for accountability purposes and as per request.
To liaise with the various government agencies, Agency for Integrated Care, restructured hospitals, and other social service / healthcare organisations with regards to cases, transport co-ordinations, and medical appointments.
To assist in the day-to-day operations and maintenance of the Medical Social Work Department.
To provide support to Medical Social Workers (e.g. case management, social-emotional support / practical assistance for residents/next-of-kin).
To undertake full spectrum of case management from application, intake assessment interviews, admissions to follow-up with discharged residents into the community.
To process applications and request for subsidies, means testing and the various financial assistance schemes.
To attend to public enquires on nursing home services and provide relevant information and referral to them.
To undertake any other ad hoc duties as assigned by the Management and/or Medical Social Worker.
Requirements:
Minimum GCE A Level or Diploma in Social Services or equivalent (for candidates without experience).
Minimum GCE O Level or equivalent with at least 1 to 2 years of experience in social service or healthcare sector.
Excellent communication and interpersonal skills, with the ability to engage residents, families, and external stakeholders.
Good knowledge in Microsoft Office skills and confident in handling documentation and records.

Willingness to learn, humility, and a positive, hands-on attitude.
