As a Social Work Associate, you will be responsible for the following duties:
- Perform assessment in financial application cases.
- Assisting patients in national financial schemes application.
- Assist medical social worker in reviewing assessing cases.
- Work together with medical social worker in providing brief social assessment.
- Collecting and organizing information for case assessment.
- Perform home visits when necessary.
- Educating patients and family members.
- Act as mentors to new Social Work Assistant within the team.
- Participate in quality improvement projects when necessary.
- Any other duties within the team.
Requirements:
- Min. Diploma in Social Work or equivalent.
