Senior Staff Nurse (Senior Care Centre)

Key Responsibilities
Clinical Assessment & Care Planning
. Conduct comprehensive admission and periodic assessments using standardizedtools (e.g., Modified FRAT, FAST).
. Develop, implement, and update individualized care plans via the Elder &Community Caresuite.
. Perform critical reassessments following hospitalizations, falls, orsignificant absences.

Medication & Clinical Management
. Oversee medication management, including preparation, packing, andadministration.
. Ensure strict adherence to the IDDSI framework for residents with swallowingdifficulties (dysphagia).
. Respond promptly to medical emergencies and maintain rigorous infectioncontrol standards.

Leadership & Supervision
. Supervise and train care staff in delegated nursing procedures, enteralfeeding, and medication support.
. Liaise with the Centre Lead regarding nursing equipment and day-careoperations.
. Lead quality improvement initiatives to enhance resident safety andwell-being.

Coordination & Communication
. Collaborate with a multidisciplinary team to ensure seamless care coordination.
. Maintain effective clinical communication with families regarding clientconditions and care goals.
. Facilitate internal and external referrals to support holistic residentneeds.
