Job Description:
- Provide palliative nursing care in hospital wards and nursing homes.
- Assess, plan, implement, and document patient care with the palliative team.
- Manage medications and syringe drivers.
- Support ward and nursing home staff on end-of-life care.
- Educate caregivers and nursing staff on comfort care and dying process.
- Coordinate care plans and end-of-life transitions to nursing homes or hospices.
- Act as liaison between hospital, nursing home, and palliative care teams.
- Participate in case discussions and provide training to nursing home staff.
Requirements:
- Min. Diploma in Nursing / Degree
- Has valid SNB / BCLS
- Relevant clinical experience.
- Good communication and interpersonal skills.
