Job Summary:
As SSN at St Luke's Community Clinic, you will be responsible for delivering high-quality, person-centered care within a dynamic GP+ environment. This role is designed for a clinical practitioner, who excels in managing multi-morbid geriatric patients and is passionate about shifting the healthcare needle from reactive treatment to proactive, community-based wellness.
The successful candidate will bridge the gap between hospital and home by overseeing day to day nursing operations, empowering caregivers through structured training, and driving long-term health literacy. Working under the guidance of the outpatient Assistant Medical director and accountable to the Director of Nursing, you will play a vital role in ensuring our patients age with dignity and receive seamless, integrated care that aligns with the national Healthier SG vision.
Responsibilities and Duties:
Operations & Resource Management
- Support in Clinic Governance: Oversee daily operations of the Outpatient Clinic (OPC) to ensure clinical efficiency and seamless patient flow.
- Material Logistics: Oversee inventory management of consumables and equipment; evaluate new medical equipment and manage requisition to ensure zero service disruption or risk to patients and staff.
- Operational Collaboration: Work with Care & Counselling, Day Rehab and Home Care Services as required to maintain continuity and high standards of care is rendered.
- Team Building: Foster a collaborative team environment and provide constructive feedback to peers and supervisors to drive service excellence and accountability.
Quality, Safety & Data Governance
- Clinical Governance: Ensure strict adherence to Infection Control policies and maintain a safe, sterile environment for all patients.
- Digital Competency: Expertly navigate NGEMR and Clinic Assist for clinical documentation while maintaining absolute PDPA compliance and data integrity.
- Quality Improvement: Participate in Nursing/Hospital Quality Assurance (QA) and Work Improvement Programs (WIP).
- Evidence-Based Practice: Utilise and contribute to nursing research to drive best practices in community geriatrics.
- Administrative Support: Assist in ensuring accurate entry and billing of nursing procedures and investigations by staff and Outpatient service assistants.
Clinical Growth & Professional Development
- Technical Clinical Advancement: Maintain a proactive commitment to continuous upskilling in advanced nursing interventions to meet the evolving clinical needs of a GP+ clinic. This includes achieving and maintaining competency in: Specialised Procedures: Male IDC (catheterization) insertion and management, and PEG tube replacement/management; and Palliative Care: Providing clinical and psychosocial support for patients requiring community-based palliative and end-of-life care.
- Collaborative Leadership & Training: Excellent interpersonal and communication skills to facilitate seamless multi-disciplinary collaboration and lead effective training programs that empower caregivers in home-based care.
- Mentorship & Training: Precept, orientate, and guide new or junior nursing staff on clinic protocols and hospital policies.
Job Requirements:
- Qualifications: Registered Nurse with a valid practicing certificate from the Singapore Nursing Board.
- Specialisation: Must possess an Advanced Diploma in Chronic Disease Management, Gerontology, or a relevant advanced clinical specialty Eg. Lifestyle coaching certification
- Experience: A minimum of 10 years of clinical experience, with a significant focus on community geriatrics and/or primary care settings.
Core Competencies:
- Complex Care Management: Demonstrated expertise in developing integrated care plans for patients with multi-morbidity, focusing on proactive health management within the Healthier SG framework.
- Advanced Geriatric Assessment: Advanced clinical assessment skills specifically adapted for the elderly, with the ability to identify subtle changes in functional, cognitive, and physical health to prevent acute decline.
- Behavioral Change & Health Literacy: Proficient in motivational interviewing (MI) and patient-coaching techniques to drive health literacy, improve self-care capabilities, and ensure long-term adherence to personalised healthier SG goals.
