Responsibilities
- Supervise and guide care staff, ensuring patient care is delivered according to established protocols and standards.
- Deliver comprehensive client care, including addressing medical emergencies, managing medications, and ensuring strict adherence to nursing, infection prevention, and safety protocols.
- Maintain accurate and detailed client medical records, including schedules for medical follow-ups and necessary documentation.
- Contribute to the development and execution of clients Individual Care Plans (ICPs), ensuring personalized and effective care.
- Conduct training for care staff and provide health education to clients, enhancing overall care quality and patient outcomes.
- Administer medications as prescribed, upholding safety standards, and maintaining precise records to ensure accountability and patient safety.
- Facilitate patient autonomy by setting individualized care objectives and educating patients and their families on medical conditions, medications, and self-care skills.
- Ensure timely submission of incident reports to the Nurse Manager, adhering to the stipulated timelines and protocols.
- Assist residents with Activities of Daily Living (ADLs), including personal hygiene, toileting, feeding, and ambulation, as needed.
- Employ clinical judgment to assess and manage patients pain levels, implementing evidence-based interventions and continuously evaluating outcomes to optimize pain management strategies.
- Collaborate with therapists to execute planned therapies, ensuring alignment with predefined goals and contributing to patients overall rehabilitation and recovery.
- Engage proactively with residents, fostering positive interactions and a supportive care environment.
- Observe, document, and report any changes in residents conditions to ensure timely and appropriate interventions.
- Maintain the cleanliness, operational readiness, and proper storage of medical equipment, instruments, and supplies, ensuring that the facility meets high standards of hygiene and safety.
- Uphold meticulous standards in all aspects of care and facility management to contribute to a safe, conducive, and efficient healthcare environment.
- Perform other duties as assigned.
Requirements
- A Registered Nurse with a valid practising certificate from the Singapore Nursing Board (SNB)
- Hold a valid Basic Cardiac Life Support (BCLS) and AED certificate and be able to perform acute emergency care if necessary
- Provide first aid to patients who require medical attention
- Possess knowledge of nursing experience and a robust clinical background in Hospitals, Transitional Care Facilities, Nursing Homes, or Day Care Centres
- Possess a professional demeanour and conduct themselves with integrity and respect towards all patients, colleagues and other parties
- Demonstrate patience and approachability while assisting patients and handling challenging situations effectively
- Demonstrate effective communication skills, both verbally and in writing, to interact with patients, colleagues and other parties
- Display adaptability and flexibility to work efficiently in diverse settings with changing demands and schedules
- Familiarity with regulatory guidelines and compliance standards.
- Possess good interpersonal and communication skills, with a positive work attitude and strong commitment towards service excellence
- Be a mature and objective leader, possess leadership qualities and positive interpersonal skills.
- Ability to work in a team and a fast-paced environment
- Ability to work on weekends and public holidays when needed
- Proficient in the use of Microsoft Office applications
