About the Role
We are looking for a compassionate and experienced Senior Physiotherapist to join our team. In this role, you will deliver patient-centred rehabilitation services across home-based and day rehabilitation settings, supporting individuals in achieving optimal functional independence and quality of life.
Key Responsibilities
- Conduct comprehensive initial assessments using appropriate clinical outcome measures
- Develop and implement individualised care plans with clear treatment goals
- Deliver physiotherapy interventions in home-based and/or day rehabilitation settings
- Provide caregiver education, training, and equipment recommendations
- Support discharge planning and coordinate referrals to relevant services where required
- Participate in monthly home therapy meetings and case discussions
- Contribute to therapy in-service sessions and attend departmental training programmes
- Perform necessary administrative and documentation duties
Requirements
- Degree or Diploma in Physiotherapy recognised by the Allied Health Professions Council (AHPC)
- Full registration with the AHPC
- Strong interpersonal and communication skills
- Ability to work independently in a home-based care setting
- Proven experience in assessment, intervention planning, and delivery of personalised care plans
