Responsibilities:
- Assess, diagnose, and provide appropriate therapy interventions for patients with various conditions within the primary care setting.
- Develop individualized treatment plans and implement suitable therapy modalities based on patients medical conditions and functional needs.
- Monitor and evaluate patients progress using appropriate outcome measures and adjust treatment plans accordingly.
- Educate patients, caregivers, and family members on rehabilitation, preventive care, and continued management of conditions.
- Conduct health education talks and community outreach programmes focusing on fall prevention and overall wellness.
- Collaborate with multidisciplinary healthcare teams to ensure holistic patient care and effective treatment outcomes.
- Contribute to continuous quality improvement initiatives and service enhancement projects within the department.
- Provide supervision, guidance, and training to students, trainees, and junior staff members.
What we're looking for:
- Min. Bachelor's Degree in Physiotherapy from a recognized university.
- Registered with AHPC
- Ability to work well independently in a challenging environment alongside a multi-disciplinary team.
- Ability to demonstrate and articulate sound clinical knowledge and reasoning, critical thinking, judgement, and decision-making in the provision of care.
- A patient-focused individual with a genuine desire to make a difference in community healthcare
