Job Description

Brief Summary

Join a dynamic team as a pharmacy leader, where you'll drive medication safety and operational excellence while enhancing patient outcomes through expert training and support.

Responsibilities

  • Supervise and mentor pharmacy teams to ensure efficient coordination of medication supply workflows for timely, accurate, and compliant dispensing. Investigate and address medication supply errors, implementing corrective solutions to prevent future occurrences. Guide teams in performing thorough medication reconciliation services, resolving discrepancies with prescribers, and accurately documenting outcomes. Proactively identify potential medication safety risks, collaborating with clinical teams to develop and implement effective action plans. Provide medication-related counseling and training for patients, the public, and healthcare staff, ensuring best practices are followed. Recommend evidence-based, cost-effective medication regimens and appropriate monitoring strategies to healthcare professionals. Lead discharge planning initiatives, assisting teams with self-administration programs and pill-box packing to enhance medication adherence post-discharge. Oversee inventory management processes, ensuring compliance with regulations through effective receipt, storage, monitoring, and destruction protocols. Manage procurement processes encompassing sourcing, price negotiations, contract administration, and supplier relations. Monitor contract compliance, including expirations and renewals, while analyzing consumption trends and pricing benchmarks. Contribute insights for medication budgeting and cost-containment strategies, focusing on waste minimization. Serve as a resource expert for pharmacy staff, providing mentorship and participating in multidisciplinary research efforts. Assist in the development of competency evaluations and training programs for clinical teams to enhance operational effectiveness. Support quality assurance initiatives by designing, auditing, and analyzing programs to assess outcomes. Facilitate management of clinical information systems, aid in enhancements, provide staff training, and conduct user acceptance testing. Manage medication-related databases, ensuring accurate creation, updates, and maintenance for informed clinical decision-making. Promote risk awareness among staff and engage in hospital-wide risk management initiatives related to pharmacy operations.

Requirements

  • Degree in Pharmacy. Registration with the Singapore Pharmacist Board, holding a valid patient care practicing license. Minimum of 5 years of demonstrated experience as a pharmacist in relevant responsibilities. Strong leadership and mentoring skills to guide pharmacy teams effectively. Ability to effectively communicate complex information to patients and healthcare professionals. Proven analytical skills in evaluating medication management and safety processes. Experience with procurement processes and inventory management within a healthcare setting.
