Job Summary
Support nursing operations by assisting Registered Nurses in supervising care staff, delivering resident care, monitoring health status, and coordinating medical appointments to ensure high-quality, safe, and compassionate care in a therapeutic environment.
Responsibilities
- Comply with approved nursing practice standards and organizational policies to ensure quality care delivery
- Assist the Registered Nurse (RN) in supervising care staff and healthcare attendants to maintain care standards
- Provide basic nursing care to residents, ensuring comfort and dignity
- Observe, monitor, and report changes in residents conditions and care needs promptly
- Deliver nursing interventions according to individualized care plans to meet resident needs
- Conduct nursing rounds jointly with the RN to assess resident status and care effectiveness
- Supervise care staff in maintaining residents personal hygiene and comfort
- Oversee care staff in accurately checking, monitoring, and recording residents vital signs
- Maintain accurate and timely documentation of all care activities and resident information
- Assist the RN in medication administration, ensuring adherence to safe medication practices
- Guide care staff in safely transferring, positioning, and mobilizing non-ambulant residents
- Supervise care staff during diet serving and feeding for residents requiring assistance
- Assist in admitting, assessing, and orienting new residents to the facility
- Perform basic rehabilitative activities such as range of motion exercises and assisted ambulation
- Arrange and escort residents to prescribed therapies, including occupational therapy sessions
- Coordinate and schedule residents medical appointments and transportation logistics
- Accompany residents to hospitals or polyclinics for follow-up appointments as needed
- Initiate appropriate emergency actions and promptly escalate issues to the RN
- Collaborate with the RN to maintain a safe, clean, and therapeutic environment within the home
- Participate in clinical service improvement projects to enhance care quality
- Actively engage in infection prevention and control initiatives to safeguard residents and staff
- Support training, guidance, and development of care staff to improve team competencies
- Perform additional duties as assigned by Senior Nurse Clinician, Director of Nursing, or CEO
Required competencies and certifications
- Locally Trained National ITE Certificate in Nursing
- Foreign Trained Diploma/Degree in Nursing
- Enrolled Nurse (EN) registration with Singapore Nursing Board
