We are seeking a skilled and proactive Senior/ Dietitian (Locum) to join the Allied Health team. Reporting to the Assistant Head, you will provide evidence-based clinical nutrition care and support department quality improvement efforts.. Your responsibilities are as follows:

Job Role

To review and provide nutritional advice to dialysis patients.

Locum Services: 16 hours / week (+- 2 hours)
Dialysis Centre coverage: 4 - 5 DCs

Main Responsibilities

1) Perform nutrition screening with reliable screening tools to evaluate patient risks, level of criticality and to direct services while adhering to the Standards of Practice (SOP) in Nutrition Care.

2) Conduct nutritional assessments with suitable methods which may include

- identify and record medical, anthropometry, biochemical, social and environmental data necessary to plan nutritional management

- perform diet interviews to obtain a comprehensive diet history,

- conduct Subjective Global Assessment (SGA),

- participate in the interdisciplinary comprehensive assessments.

3) Integrate foundational dietetics knowledge with critical appraisal of assessment data to diagnose nutrition problem(s) which can be resolved or improved through treatment or nutrition intervention.

4) Develop nutrition plans and implement interventions (e.g. diet counselling) based on knowledge of patient's current health and nutrition problem(s).

5) Provide client follow-up and modify nutrition care plan as necessary.

6) Involved in continuous planning, execution and improvement of nutrition projects and programs (e.g. outreach program, group education program, etc)

7) Review literatures and/or utilize appropriate educational resources (e.g. workshop, conferences) to update knowledge and develop skill.

8) Perform other duties as assigned.

Qualification

Bachelor or advance degree with major studies in food and nutrition or dietetics and completed clinical dietetics internship (at least 24 weeks) in approved institution A member of Singapore Nutrition and Dietetic Association and obtained yearly certificate of Accredited Dietitian in Singapore (advisable)

Experience

1 year experience in clinical nutrition (preferred)
