Responsibilities:
- Develop and enhance care coordination workflows with clinical and operational stakeholders.
- Lead case management initiatives and support integrated, patient-centred care delivery.
- Assess patient needs, identify care gaps, and coordinate appropriate interventions and referrals.
- Facilitate care planning discussions with patients and caregivers to support care transitions and community-based care.
- Coordinate and triage referrals for transitional and community care services.
- Monitor patient progress and ensure timely follow-up for continuity of care.
- Maintain accurate clinical documentation, referral records, and programme data.
- Collaborate with healthcare professionals, community partners, and relevant agencies to strengthen care integration.
- Provide guidance and supervision to junior care coordination staff.
- Participate in quality improvement, research, training, and community engagement initiatives.
Requirements:
- Degree or equivalent professional qualification in Nursing, Social Work, Allied Health, or a related discipline, with at least 6 to 8 years of healthcare experience in case management, care coordination, nursing, or related functions.
- Experience working with multidisciplinary healthcare teams, with leadership, supervisory, programme development, or service improvement experience being an added advantage.
- Knowledge of acute, community, home-based, rehabilitative, psychological, specialised medical, or surgical care services is an added advantage.
- Strong stakeholder management, communication, interpersonal, organisational, analytical, and problem-solving skills, with the ability to work independently in a dynamic healthcare environment.
- Proficient in Microsoft Office applications familiarity with healthcare information systems is an added advantage.
- Proficiency in local languages and dialects is an advantage for liaising with local languages-speaking and dialects-speaking patients, caregivers, and stakeholders.
