Job Summary
The Programme Coordinator/Executive supports social work and community care services by collaborating with social workers, volunteers, and partners to coordinate care, facilitate service access, and ensure effective programme and case plan implementation.
Responsibilities
- Collaborate with social workers to implement casework interventions and follow-up actions that improve client well-being
- Execute care plans to deliver timely and appropriate support services to clients
- Coordinate medical escort services to ensure safe and efficient client transportation to medical appointments
- Maintain client's data base and accurate case documentation, including case recordings and social reports, to support client management
- Facilitate intake processes, conduct basic assessments, and coordinate referrals to connect clients with needed services
- Develop and sustain relationships with monastics, volunteers, and community partners to strengthen community support networks
- Support outreach initiatives to increase engagement and identify client needs for service connection
- Provide administrative and logistical support for community programmes, events, and fellowship activities to ensure smooth execution
- Deliver general administrative support to the team to facilitate daily operations
- Assist management with ad-hoc assignments and special projects to support organisational goals
- Adapt to support weekend activities or events as required to meet programme needs
Qualifications and Skills - Diploma in any discipline qualifications in Community Care or Social Services will be an advantage.
- 2-3 years of relevant experience in healthcare, social services, or administrative roles preferred fresh graduates are welcome to apply.
- Proficient in Microsoft Office (Word, Excel, PowerPoint).
- Strong interpersonal and communication skills (written and verbal).
- Very good command of Mandarin (Chinese), including reading, writing, and speaking, is required to effectively communicate with monastics English is secondary.
- Empathetic, compassionate, and comfortable working with elderly clients.
- Well-organised, detail-oriented, and able to manage multiple tasks effectively.
- Able to work independently as well as part of a team.
- Willing to work on weekends when required.
