Role Overview
We are looking for a passionate and dynamic Programme & Case Management Associate who will support adults (aged above 45) with cancer through case management, programme development, and community engagement.
This role is ideal for someone who enjoys people-facing work, is able to multi-task, and thrives in a fast-moving, mission-driven environment.
Key Responsibilities
1. Case Management & Client Support
- Provide direct psychosocial support to young adults with cancer.
- Conduct needs assessments and ongoing follow-ups.
- Develop personalised case management plans and track progress.
- Coordinate referrals to the community or healthcare services as needed.
- Maintain proper documentation, case notes, and confidentiality in line with organisational and sector standards.
2. Programme Development & Facilitation
- Co-develop and run community programmes and events for different segments of our client group
- Build youth-friendly, engaging, and evidence-informed programme content.
- Support programme evaluation, data collection, and reporting.
- Assist in volunteer coordination during programmes.
3. Community Engagement & Stakeholder Relations
- Represent the organisation at outreach events, hospital fairs, and community sessions.
- Work collaboratively with healthcare professionals, social workers, counsellors, and programme partners.
- Support communications (e.g., WhatsApp Groups, outreach materials) where needed.
- Support organisational logistics, administration, and ad-hoc tasks.
Requirements
Education & Experience
- Diploma or Degree in Social Work, Psychology, Counselling, Community Development, Youth Work, Public Health, or related fields.
- Prior experience with health-related support, oncology, or community social services is advantageous.
- Fresh graduates with strong passion and relevant internships will be considered.
- Mid-career professionals with relevant and related certifications in social service will be considered.

- Comfortable handling casework, crisis support, and multi-stakeholder communication.
- Good presentation and facilitation skills.
- Excellent organisational and multi-tasking abilities able to work with some ambiguity.
