Program Coordinator /Patient Service Associate (Home Care Team, 2 shifts) #HKN
Location: West
Working hours: Any 5 days per week, including both weekends & PH.
2 Rotating shifts (by roster): 8am - 6pm 12.30pm - 8.30pm
Salary: up to $3000, $18/hr for locum roles
Job Responsibilities
- Support planning, assessment, and ongoing monitoring of programme operations to ensure high-quality patient care.
- Coordinate with external vendors to arrange patient transport and courier services.
- Liaise with hospital bed management units to facilitate bed requests and allocation.
- Coordinate with multiple departments to schedule appointments and ancillary services as requested by clinicians.
- Manage patient admissions, discharges, and transfers within the Electronic Medical Record (EMR) system.
- Provide financial counselling to patients and next-of-kin on estimated bills, available subsidies, payment schemes, and modes of payment.
- Assist with completion of payment documentation and ensure proper follow-up for cases through to discharge.
- Generate and export reports from EMR systems and update relevant databases to support clinical operations.
- Maintain accurate documentation and ensure timely information flow across teams.
Job Requirements
- Min Diploma in any discipline.
- Proficient in Microsoft Office applications and computer systems.
- Willing to work beyond regular hours, including weekends and public holidays when required.
- Able to thrive in a fast-paced environment and manage multiple tasks effectively.
- Strong team player with a positive and proactive work attitude.
- Prior experience in a hospital or healthcare setting is an advantage.
- 6 months Locum positions are available.
