- To assess, plan, implement, evaluate and document therapy requirements of patients to facilitate, restore, improve or maintain movement/function for all new admissions.
- Review and update plan 6 monthly or more frequently as needed
- Lead therapy staff to ensure safe and quality patient care, and in compliance to standards. Is responsible for the supervision, evaluation and disciplining of staff in her/ his team
- Adopt evidence-based practices on therapy services
- Maintain written policies, procedures and work process are available and up to date to guide therapy services
- Ensure that therapy services are in accordance to policies, guidelines and procedures
- Support implementation of patient's discharge plan to ensure smooth transition back to the community
- Train caregivers in preparation for patient's discharge
- Monitor therapy service quality KPIs and develop action plan for continuous improvement
- Encouraging exercise and movement to improve or maintain remaining abilities
- Review and update therapy services workflow, policies and procedures
- Ensure team members are familiar with SOP and all emergency protocols
- Work with multi-disciplinary teams to coordinate, conduct and facilitate quality improvement initiatives and continuous improvement projects
- Maintain systematic inventory control and equipment maintenance to achieve optimal utilisation and ensure safe usage
- Manage and evaluate therapy services to provide safe, effective and quality care for patients
- Manage therapy aides development, skills and competency upgrading through various opportunities
- Educate and supervise therapy aides in performing the various prescribed therapy services
- Continuous assessment of therapy aides competencies to ensure achievement of expected competency levels
- Orientation and appraisal of newly joined staff members
- Assist Senior PT to identify, plan training programs for therapy aides development
- Motivate staff to perform at their optimal level and ensure care delivered is in compliance to standards of practice
- Ensure proper storage of all confidential files, records, documents to ensure patients confidentiality
- Maintain cohesive working relationships within Therapy team with internal and external customer stakeholders
Requirement
- At least with 2 year work experiences or above.
- Candidates with 5+ years of physiotherapy experience within health care setting will be considered for senior position.
- Full AHPC registration
