Job purpose
- To provide client-centred physiotherapy services that improve clients mobility, function, independence, and quality of life in home and community settings.
Key responsibilities
- Conduct comprehensive assessments of clients physical, functional, and mobility status.
- Develop and implement individualised physiotherapy intervention plans based on clinical findings and client goals.
- Provide evidence-based therapy to improve strength, balance, endurance, and movement efficiency.
- Educate clients and caregivers on exercise programmes, fall prevention, and safe mobility techniques.
- Document assessment findings, treatment plans, progress, and outcomes accurately and in a timely manner.
- Collaborate with multidisciplinary team members for coordinated and holistic client care.
- Participate in client case conference, community outreach, and health promotion activities.
- Supervise therapy assistants
- Participate in clinical student placement
- Participate in internal training
- To provide service to various settings (e.g. Home Rehab, CR, CH, NH) as deemed competent
Qualifications and Skilled Required
- Diploma/Degree in Physiotherapy
- Full Registered with the Allied Health Professions Council in Singapore
