Job Description:
- Dispensing and educating patients on their medications, medication review and reconciliation, therapeutic interventions and recommendations, multidisciplinary rounds and collaborative prescribing for qualified persons.
- Recommend and monitor drug therapy to achieve desired outcomes.
- Provide drug information to other healthcare professional and patients.
- Identify, resolve and prevent drug-related problems.
- Provide pharmaceutical care, working collaboratively in a multidisciplinary team in various clinical settings, including outpatient, inpatient and community settings.
- Ensure pharmacy policies and procedures are implemented consistently.
- Other ad hoc duties assigned
