Responsible for delivery of patients service in the work area in a personalized, specialized and cost-effective way in accordance to established hospital policies, procedures and standard of practices.
Primary Responsibilities and Duties
- To assess for mental health issues, provide case formulation and recommendations for treatment.
- To provide appropriate counseling / psychotherapy for identified inpatients and outpatients.
- To contribute, as part of an inter-disciplinary team, in the overall care plan for the patient.
- As part of an inter-disciplinary team, to facilitate transition to adult-care.
- To develop or improve on workflow for better patient care.
- To initiate and implement new psycho-social programmes
- To assist in the development of materials for teaching and training.
- Provide in-service training for department staff as needed.
Requirements:
- Min. Degree in Social Work
- No exp needed - training provided
- Min 2 to 4 years of social worker experience will be an advantage
