Job Responsibilities:
- Conduct home visits to provide nursing care for patients with chronic illnesses and palliative care needs.
- Perform comprehensive nursing assessments and develop appropriate care plans.
- Coordinate with doctors on patients clinical conditions, medical assessments, and follow-up care.
- Administer and monitor prescribed medications, ensuring compliance with treatment plans.
- Monitor patients progress and implement appropriate nursing interventions.
- Educate patients, caregivers, and family members on home nursing care and disease management.
- Maintain accurate clinical documentation and update patient records using electronic systems.
- Collaborate with the multidisciplinary healthcare team to ensure continuity and quality of patient care.
- Perform other nursing duties as assigned.
Requirements:
- Registered Nurse with a valid Singapore Nursing Board (SNB) practising certificate.
- Palliative care training and experience in home care or community nursing.
- Competent in performing clinical nursing procedures independently.
