Job Description

  • This role is like management a case (case management) manage the care plan / pain management for the palliative patients
  • You will be the key person who will make the most difference as you make home visits to provide palliative care and recommend psycho-social support to our terminally ill patients and their families.
  • You will be part of the most dedicated, compassionate and caring teams comprising doctors, nurses and medical social workers providing an all-in-one professional care to patients at home at all hours, seven days a week.

Requirement :

  • State Registered Nurse with a valid Practising Certificate issued by Singapore Nursing Board (SNB) with at least 3 years of working experience
  • Experience in Oncology or Palliative Care / ICU will be an advantage
  • Nurses that are passionate with palliative case are welcome too.
  • Multi-tasker who is passionate about providing holistic care for patients in their home environment
