The Palliative Care Consultant provides specialist palliative care to patients with life-limiting illnesses across inpatient, outpatient, and community settings. The role focuses on holistic, patient-centred care, integration of palliative principles into general medical services, capacity building of the multidisciplinary team, and advancement of SLH's palliative care strategy in line with national priorities.
Responsibilities:
Clinical Care & Education
- Provide specialist palliative care consultations, including complex symptom management,
- goals-of-care discussions, and end-of-life care.
- Provide clinical leadership and mentorship to junior doctors and palliative care team members.
- Design and deliver structured education programmes in palliative care, communication skills, and end-of-life management.
- Build palliative care competencies across the hospital through regular teaching, case discussions, and interprofessional learning.
Clinical Governance & Quality Improvement
- Lead and contribute to development, review, and implementation of palliative care clinical policies, pathways, and SOPs.
- Drive quality improvement initiatives, audits, and outcome measurement related to end-of-life care.
- Support compliance with accreditation, regulatory, and clinical governance requirements.
- Promote safe, ethical, and evidence-based palliative care practices across services.
Service Development & Strategy
- Contribute to the design and implementation of palliative care service models
- Strengthen integration between hospital, community partners, home care, and hospice services to ensure seamless transitions of care.
- Participate in service evaluation, research, and grant-funded initiatives where appropriate.
- Support strategic planning to meet evolving palliative care needs in a geriatric and sub-acute population.
Requirements:
- Basic Medical Degree and postgraduate qualifications registrable with Singapore Medical Council.
- Advance specialist training in palliative medicine, will be requirement for the above position
- Committed and passionate in the care of patients who are in their end-of-life journey and those with multiple physical, psycho-emotional and social needs.
