Job description:

An Occupational Therapist is responsible for providing patient care, performing therapy execution and patient education activities to an assigned ward of patients. S/he performs continuing education and research to achieve quality patient care. S/he is also conscientious in providing therapy recommendations based on evaluation of the patients conditions.

S/he works as part of collaborative, interdisciplinary teams such as doctors, nurses, medical social workers, physiotherapists, speech therapists, care coordinators and pastoral care staff.

S/he should have initiative and be sensitive to the needs of his/her patients. S/he should possess communication and problem-solving skills

1. Key Work Functions and Tasks

a. Conduct patient assessment and therapy planning

b. Conduct and evaluate functional assessment to determine required therapy interventions

c. Formulate client-centered goals in collaboration with patients and/or their caregivers

d. Plan, perform and modify suitable therapy interventions to achieve the documented goals. These interventions include, but are not limited to :

i. Basic Activities of Daily Living (BADLs) retraining

ii. Instrument Activities of Daily Living (IADLs) retraining

iii. Functional activities and exercises to improve/maintain upper limb strength, range of motion, motor skills and coordination

iv. Cognitive stimulating activities requiring the use of various cognitive domains to improve/maintain cognitive ability of patients

v. Provision of advice on home safety and environmental modifications based on patients needs and safety considerations

vi. Recommendation and prescription of assistive devices and equipment to promote/enhance occupational engagement and performance

vii. Home visit where deemed necessary

viii. Caregiver training and education

ix. Powered mobility training and prescription

x. Group therapy

xi. Neuromuscular Electrical Stimulation (NMES)

xii. Use of selected outcome measures to monitor patients progress, review goals and discharge plans

xiii. Participate, collaborate and discuss discharge plains in multi-disciplinary ward meetings

xiv. Application of assistive devices and/or equipment prescription through subsidies eg. Senior Mobility Fund (SMF) and Assistive Technology Fund (ATF)

xv. Assess and complete Functional Assessment Reports (FAR)

xvi. Providing concise documentation and adherence to OneRehab requirements in accordance to department's documentation audit guidelines

xvii. Adherence to Allied Health Professional Council (AHPC) Code of Professional Conduct

2. Perform clinical administration and operations

a. Ensuring safe and efficient service delivery while managing ward work load

b. Oversee assigned therapy area's inventory of equipment

c. Participate in the development and implementation of new work processes as necessary

d. Participate in risk management & service quality

e. Adherence to infection control measures

f. Participate in quality improvement projects

g. Escalate incidents or adverse events to reporting officer in adherence to Guidelines for Incident Reports (Annex K)

h. Perform peer documentation audit in accordance to department's documentation audit guidelines

i. Participate in continuing education and professional development

j. Keeping abreast with current evidence in relation to Occupational Therapy practice and deliver them during in-service and training to colleagues

k. Engage actively in identifying and discussing suitable courses during annual Learning Needs Analysis (LNA) exercise with reporting officer

l. Providing supervision and team development

m. Provide clinical supervision for therapy assistants and conditional-registered therapists

n. Provide clinical supervision for students as assigned

Salary Range: $4000-$6800
