Job Description & Requirements
Clinical Occupational Therapy Services:
. Conduct comprehensive functional assessments, including activities of daily living (ADLs), mobility, upper limb function, cognition, and psychosocial status where applicable.
. Develop, implement, and regularly review individualized, goal-oriented therapy plans tailored to residents functional needs and rehabilitation potential.
. Provide evidence-based interventions, including but not limited to:
ADL retraining and functional task practice
Upper limb rehabilitation and positioning
Cognitive rehabilitation and stimulation
Sensory and perceptual training where indicated
Prescription and training in the use of assistive devices and adaptive equipment
Environmental modification to support safety and independence
. Facilitate individual and group-based therapeutic activities to promote engagement, social participation, and overall well-being of residents.
. Support falls prevention and risk reduction through functional assessment, environmental recommendations, and staff guidance.
. Provide recommendations on seating, positioning, pressure care, and safe handling techniques in collaboration with the multidisciplinary team.
. Liaise and collaborate closely with the multidisciplinary healthcare team (e.g. nursing, physiotherapy, medical, social work) to ensure coordinated and holistic care.
. Provide guidance, education, and basic training to care staff and caregivers to support carryover of therapy goals during daily care routines.
. Maintain accurate, timely, and professional documentation in designated electronic systems (e.g. ECCareSuite, Salesforce), in accordance with organizational and regulatory requirements.
. Monitor residents progress, evaluate outcomes, and adjust intervention plans accordingly to ensure continuity and effectiveness of care.
. Respond to therapy-related queries and provide clinical input between scheduled visits where required.
. Participate in case discussions, care planning, and quality improvement initiatives where applicable
AHPC-registered Occupational Therapist(s) with relevant experience in geriatric/long-term care.
