Co-lead the Rehab Therapy services, rehabilitation and wellness programmes for Residents and Day Activity Centre (DAC) clients.
Enhance clinical standards, develop therapy programmes for quality service delivery by working with the multi-disciplinary team (MDT).
Review workflows and recommend improvements in consultation with the management and rehab team.
Review and recommend appropriate wheelchairs for residents through ATF funding in consultation with Principal OT
Develop service delivery outcomes for the Rehab Department, treatment objectives for each beneficiary and implementing targeted programmes.
Oversee proper documentation, progress monitoring, re-assessments, and goal adjustments for all beneficiaries.
Maintain and coordinate asset management, including acquisition, maintenance, and repair of equipment, assistive devices, and wheelchairs.
Train and guide staff to follow evidence-based practices and maintain standards of clinical care and transfers.
Participate in Individual Care Plan (ICP) every 6 months, monitor progress made, and attend MDT meetings, and Admission & Review Committee Meetings.
Collaborate with the Volunteer Programme Management on orientation, training, and coordination of volunteers for supporting clients, including those using manual and motorized wheelchairs.
Identify suitable residents for participation in various volunteer programmes and social activities.
Develop and implement a comprehensive Model of Care encompassing Active Rehabilitation, Maintenance, and Wellness Activities for residents at SCH.
Introduce skilled interventions and ensure proper clinical assessments for all residents and DAC clients.
Able to work independently for assessment and make recommendations for residents therapy interventions
Any other relevant duties as assigned by supervisors
Job Requirements:
Possess a recognized Degree/ Diploma in Occupational Therapy with at least 3 years of experience working in similar MSF disability homes.
Full registration with the Allied Health Professions Council (AHPC)
Preferably a member of the Singapore Association of Occupational Therapists (SAOT)
