Location: Riverwalk
Working Days: 3 days/week - Monday, Wednesday & Friday
Working Hours: 8 hours/day
Contract: 2 years, renewable for another 2 years
Key Duties:
The Occupational Therapist shall be responsible for the following:
. Resident Assessment: Conduct initial, routine and ad-hoc occupational therapy assessments for residents.
. Documentation: Maintain accurate and timely clinical records, including assessment findings, treatment plans, and progress notes.
. Treatment Planning: Develop individualized, goal-oriented occupational therapy care plans based on each resident's needs and functional status.
. Direct Resident Care: Provide occupational therapy interventions, including but not limited to: rehabilitation in activities of daily living (ADLs),prescription of adaptive equipment to aid in independence where appropriate, conduct cognitive assessment and interventions, promote activity participation through engagement in activities of interests, conducting of home visits and caregiver training for purpose of discharge planning.
. Program Planning and Training: Design and implement group and individual therapy programs, and provide training to residents, caregivers, and therapy staff on exercises, safe transfers, and mobility techniques
Requirements: AHPC Full Registration with valid Practicing Certificate and 2-3 years relevant experience.
