Provide occupational therapy assessment and intervention for persons with dementia and their carers.
Develop an individualised care plan for the person living with dementia and carers to live well with dementia.
Assess the wellbeing of persons with dementia and implement person-centred strategies to address behaviours of concern and enhance activity engagement.
Conduct individual or group interventions.
Provide occupational therapy consultation, training and support to relevant members of the care team, family carers, and domestic helpers.
Provide community case management service for the person with dementia and the carer.
Adopt a transdisciplinary approach to care and work with multi-disciplinary team members for better care outcome.
Carry out any other duties and responsibilities as and when assigned.
Requirements
Diploma/ Degree in Occupational Therapy recognised by the Allied Health Professionals Council (AHPC).
Has obtained full registration with AHPC.
At least 2 years of professional practice experience.
Experience working in dementia care, geriatrics or mental health settings preferred.
