Medical Social Worker

Using a person-centric lens, Medical Social Worker provide individuals, families, caregivers and vulnerable populations with psychosocial support needed to cope with chronic and acute illnesses. Services include case management and interventions targeted at mitigating risk, promoting well-being and addressing barriers to healthcare access. This would be done through assessment and formulation of intervention plans, care coordination, providing supportive counselling, patient education, research and program development as well as providing information and referrals for other social services in the community.
