Medical Social Worker

• Location: Bishan
• Schedule: Mon - Fri | 8:30 AM - 6:00 PM

Responsibilities:

• Holistic Care: Support clients emotional, social, and spiritual needs through casework, counselling, and group activities.
• Care Transitions: Manage intake assessments, respite admissions, and seamless discharge/placement planning.
• Crisis & Support: Conduct safety/risk evaluations and support families during difficult life transitions.
• End-of-Life Planning: Lead Advance Care Planning (ACP), legacy work, and provide empathetic bereavement support.

Requirements:

• SWAAB-Registered Social Worker with a recognized Degree/Grad Dip in Social Work.
• 3-5 years experience in a hospital, community care, or eldercare setting.
