Provide and facilitate socio-economic evaluations to help patients and their families address practical concerns, such as financial matters, which are often an issue at the end of life
On-going individual and family counseling through face-to-face sessions
Work with the broader community which includes educational, religious, and social service organizations so that patients and their families can achieve the optimal care plan
Requirements
Degree or Graduate Diploma in Social Work
Registered with the Singapore Association of Social Workers
Passion to serve and provide good patient care
Good interpersonal skills and able to work independently
Flexible, adaptable, and able to work effectively in varied settings
