About the Role
As a Medical Social Worker in palliative care, you will play a critical role in addressing the psychosocial, emotional, and practical needs of patients and their families. You will work closely with doctors, nurses, and allied health professionals to ensure seamless, patient-centred care across the continuum.
Responsibilities
- Conduct comprehensive psychosocial assessments for patients with life-limiting conditions and their caregivers
- Provide counselling and emotional support, including grief, loss, and bereavement support
- Facilitate advance care planning (ACP) discussions and end-of-life decision-making
- Coordinate discharge planning and referrals to hospice, home care, and community support services
- Assist patients with financial counselling and access to healthcare subsidies (e.g. Medifund, grants, community resources)
- Support families in coping with caregiving stress, role transitions, and anticipatory grief
- Advocate for patients preferences and dignity in care planning
- Collaborate with multidisciplinary teams to ensure holistic and coordinated care delivery
- Maintain accurate case documentation and participate in case reviews and quality improvement initiatives
Qualifications
- Degree in Social Work or equivalent
- Registered Social Worker with the Singapore Association of Social Workers (preferred)
- Relevant experience in palliative care, hospice, oncology, or medical social work
Required Skills
- Strong counselling, communication, and interpersonal skills
- Ability to manage complex cases with empathy and professionalism
