About the Role
We are looking for a Medical Social Worker to provide emotional, psychosocial and practical support to residents and their next-of-kin. You will deliver casework and counselling, coordinate care with internal and external partners, and support residents to stabilise their situations and improve quality of life.
Job Responsibilities
- Assess referrals, conduct intake assessments and coordinate placement/admission applications in a timely manner in accordance with stipulated KPIs.
- Conduct means testing and social/financial assessments of residents and families process applications for subsidies and financial assistance schemes.
- Provide financial counselling and support residents/families with financial assistance applications where needed.
- Undertake full-spectrum case management: admission interviews, admissions coordination, documentation, follow-ups (including after discharge into the community where applicable).
- Collaborate with the multidisciplinary team to support residents psychosocial and emotional well-being and ensure holistic care.
- Liaise with external partners (e.g., AIC, hospitals, government agencies, social service and healthcare organisations) for case coordination, transport arrangements and medical appointments.
- Conduct Advance Care Planning (ACP) discussions with residents and/or next-of-kin.
- Maintain accurate documentation, case records, statistics and reports ensure strict confidentiality and PDPA compliance.
- Contribute to the development/review of MedicalSocial Work SOPs.
- Review cases and provide recommendations to Management.
- Undertake any other ad hoc duties as assigned.
Job Requirements
- Minimum Bachelor's Degree in Social Services or an equivalent qualification.
- Minimum 1 to 2 years of relevant working experience in the social service and/or healthcare sector.
- Good interpersonal and interdepartmental communication skills.
- Good working knowledge of BRIGHT, National MeansTesting System (NMTS), and IngotPCC systems will be an advantage.
- ACP/PPC and InterRAI training will be an advantage.
- Possess basic counselling skills (preferred).
