Medical Social Worker (Homecare)

Job Description

. Explore and provide psycho-social/emotional care and support to patients and caregivers/family.

. Lead in ACP discussions and completion. Complete Preferred Care Plan

. Provide Bereavement Support.

. Coordinate in-house Financial Assistance for needy patients.

. Coordinate Medical Escort & Transport for patients.

. Coordinate in referring patients for Respite Care, Nursing Home and other necessities.

. Counselling/ Reconciliation

. Crisis Intervention

. Education, Teaching & Networking

Job Requirements

. Degree or postgraduate diploma in Social Work

. Preferably 2 years relevant experience

. Must have interest to work with elderly

. Displays compassion, a positive and caring attitude towards patients, internal and external clients.

. Excellent communication, interpersonal and problem solving skills

. Meticulous, diligent and a good team player.

. Adaptable and comfortable with changes
