Medical Social Worker (Home Care)

• Responsible for case work management involving systemic assessment and intervention for the well-being of patients and family members.
• Perform referrals to other service providers and conducts home visit where necessary.
• Work closely with multi-disciplinary team members.
• Support projects and programmes.
• Attend training and case work supervision.

Job Requirements:

• Degree in Social Work or a Graduate Diploma in Social Work.
• Registered with Singapore Association of Social Work.
• Experience in means testing and healthcare setting is advantageous .
• Salary range: $3400 - $5600
• Working location: Toa Payoh
• Working hours: Monday to Thursday: 8.00am to 5.30pm/Friday: 8.00am to 5.00pm
