About the Company

The Medical Social Worker (MSW) is a trained professional responsible for delivering psychosocial care, complex case management, and clinical interventions to residents and families within a nursing home setting. The MSW leads care planning for residents with significant psychosocial, medical, or family dynamics. He/she is also responsible for ensuring service delivery aligns with national frameworks, organizational policies, and professional ethics.

About the Role

The MSW is responsible for delivering psychosocial care, complex case management, and clinical interventions to residents and families within a nursing home setting.

Responsibilities

  • Lead case management and psychosocial assessment of residents and families, particularly in complex, high-risk, or high-profile cases (e.g., elder abuse, suicidality, MSF referrals, family breakdown).
  • Apply systemic Biopsychosocial-Spiritual (BPSS) frameworks to identify intervention needs.
  • Formulate psychosocial care plans with interdisciplinary teams.
  • Prepare social reports, ensuring clinical quality and documentation standards are met.
  • Conduct grief support, family mediation, and mental health interventions when appropriate.
  • Collaborate in case conferences/discussion with other agencies.
  • Escalate cases to Supervisor for review and guidance; supervise SWAs in case follow-through.
  • Support the admission process of residents assigned by AIC for all ECON Medicare Centres and Nursing Homes, including preadmission counselling.
  • Work with referral sources to coordinate admission matters.
  • Put up applications to support resident's needs for other schemes and services, and nursing home conversions.
  • Work with National and Organisational systems to maintain bed and application status.
  • Perform National Means Testing (NMTS FA) for residents and ensure the NMTS is valid and updated according to MOH requirements.
  • Assist residents in applying for financial schemes.
  • Prepare for Medifund administration.
  • Support the development of psychosocial programmes to improve social care outcomes for caregivers and residents.
  • Partner with stakeholders to co-develop programmes inputs.
  • Conduct programme activities and/or workshops.
  • Involve and participate in continuous learning through in-house and external training programmes.
  • Participate in regular social work supervision and group training.
  • Maintain accurate documentation of case notes, financial assessments, and intervention records in compliance with PDPA and organizational policies.
  • Support departmental administration and reporting when required.

Qualifications

  • Bachelor's Degree in Social Work (BSW).
  • Registered or eligible for registration with the Singapore Association of Social Workers (SASW)

Required Skills

  • Competence in case formulation, clinical documentation, and application of subsidy frameworks.
  • Competent in IT skill
