As a Medical Social Worker (Community Services), you will be responsible for the following duties:
- Conduct comprehensive psychosocial, functional, medical, and financial assessments for individuals and their caregivers using standardised frameworks.
- Develop and implement personalised care plans with coordinated follow-through to address identified needs.
- Collaborate with interdisciplinary teams across health and community sectors to ensure seamless, continuous care.
- Provide counselling, emotional support, and psychoeducation to support coping with illness, loss, and life transitions.
- Manage crisis interventions and facilitate timely referrals for complex or high-risk cases.
- Conduct home-based visits independently or as part of a coordinated team.
- Maintain accurate, timely, and comprehensive case documentation in line with professional standards.
- Contribute to quality improvement, service development, and programme evaluation initiatives.
- Support community outreach, public education, and awareness efforts to promote programme enrolment.
- Pursue continuous professional development through training, supervision, and peer case discussions.
Requirements:
- Possess a Degree in Social Work/ Post Graduate Diploma in Social Work or equivalent.
