Provide counseling service to patients and care givers
Provide psychosocial and financial assessment, coordinated case management and interventions to enable the patient/client to function optimally in the home environment and community at large
Assist patient to understand the diagnosis and implication on ADL functioning
Walk through with patients and care givers and provide emotional support and motivation for them
Manage patients expectation and set realistic and achievable goals
Arrange for interim care or make referrals for long term step-down facilities when necessary
Participate in ward rounds and case conferences to represent patients social, psychological and economic circumstances.
Responsible for Volunteer Management
Plan and oversee the volunteer programs, not limited to recruitment, screening, coordination, training and retention of volunteers
Undertake projects and any other duties as assigned by the Management
Requirements
Master/Bachelor/Post Graduate Diploma in Social Work
Registered Social Workers with Singapore Association of Social Work (SASW) - will be an advantage
Compassion and care for patients
Good team player with positive attitude, caring and compassionate individual
Good interpersonal skills, able to communicate people at all levels such as patient's families, government agencies and other professionals
