Provide counselling service to patients and caregivers
Provide psychosocial and financial assessment, coordinated case management and interventions to enable the patient/client to function optimally in the home environment and community at large
Assist the patient to understand the diagnosis and implications on ADL functioning
Walk through with patients and caregivers and provide emotional support and motivation for them
Manage patients expectations and set realistic and achievable goals
Arrange for interim care or make referrals for long-term step-down facilities when necessary
Participate in ward rounds and case conferences to represent patients social, psychological and economic circumstances.
Undertake projects and any other duties as assigned by the Management
Requirements:
Master/Bachelor/Post Graduate Diploma in Social Work
Registered Social Workers with the Singapore Association of Social Work (SASW) - will be an advantage
Compassion and care for patients
Good team player with positive attitude, caring and compassionate individual
Good interpersonal skills, able to communicate with people at all levels, such as patients families, government agencies and other professionals
