As a social worker, you'll play a key role in providing holistic psychosocial support to renal patients and their families. This position is responsible for assessing needs, facilitating access to financial and community resources, and supporting patients emotional well-being throughout their treatment journey. Working closely with the multidisciplinary team, you will ensure coordinated care planning, promote patient resilience, and contribute to program and service development that enhances quality of life for patients.
Job Responsibilities
- Oversee all aspects of case management for renal patients, from initial referral and intake to discharge planning. This includes conducting home visits, performing risk assessments, and creating comprehensive care plans.
- Conduct means testing and financial counselling, assisting patients and their families with financial aid applications. Provide psycho-social and emotional support to ensure patient well-being is effectively addressed.
- Perform bio-psycho-social-spiritual (BPSS) and financial assessments to identify patient needs and determine necessary interventions and support.
- Liaise with hospitals, government bodies, and social service agencies to coordinate patient placements and mobilize resources like transport and support services.
- Maintain timely and accurate documentation of case notes and prepare periodic reports on social services.
- Co-develop, plan, implement, and evaluate programs both center-based and outreach that support the physical, emotional, and social needs of patients.
- Support patient engagement and empowerment through educational, wellness, and psychosocial programs.
- Assist in training and supervising volunteers, ensuring they are well-equipped to provide socioemotional support to patients. Engage volunteers in center events, projects, and outreach activities.
- Work with the management team to develop, review, and update SOPs related to admissions, financial aid, and social services.
- Contribute to the strategic planning for Tzu Chi's renal care initiatives by providing insights and recommendations based on frontline experience and social work expertise.
Job Requirements:
- Degree, Graduate Diploma or Diploma in Social Work from a recognized institution.
- 2- 3 years of relevant experience in the community healthcare or dialysis centre will be an added advantage.
- Proficient in MS Office applications, particularly Microsoft Excel, Word and Outlook.
- Good command of spoken and written English and Chinese to communicate effectively with non-English-speaking patients and volunteers.
- Strong communication and interpersonal skills, with the ability to work independently and as part of a team.
- Demonstrates empathy, compassion, and a strong commitment to serving the needy.
