Case Management and Counselling

Admission
Assess social and financial situation of potential residents and families to determine suitability for nursing home placement, with appropriate guidance. Provide pre-admission financial counselling and coordinate admissions.

Psychosocial Assessment
Assess coping, strengths and resilience of residents and families. Monitor psychosocial well-being within assigned wards and highlight concerns to supervising staff where necessary.

Counselling
Provide supportive counselling to residents and families with psychosocial needs.

Facilitation
Support and facilitate family conferences (e.g. Advance Care Planning, care discussions, financial matters).

Risk Assessment
Identify and flag potential risks (e.g. self-harm, behavioural concerns, family-related risks) and escalate appropriately.

Financial Assessment & Assistance
Conduct financial assessments and applications for assistance schemes (e.g. Medifund, MFEC, AIC LTCS).

Programme Implementation
Support departmental initiatives, resident engagement activities, and community programmes.

Discharge Planning
Work with multidisciplinary teams to support discharge planning.

Information & Referral
Link residents and caregivers with appropriate community resources and services.

Documentation & Reporting
Maintain accurate and timely case documentation and progress notes.

Job Requirements

Educational level

  • Diploma qualifications in social work or Bachelor Degree in relevant fields (Counselling, Psychology, Sociology, etc.).
  • Candidates with relevant hands-on experience in casework or social service settings will be considered.

Experience level

  • Experience in case work, client engagement, or social service delivery
  • Demonstrated ability to conduct assessments and support care planning
  • Experience working with individuals and families facing psychosocial needs is an advantage
  • Familiarity with community resources and inter-agency coordination

General Requirements

  • Willing and keen to work with elderly
  • Able to work independently with supervision and within a multi disciplinary team
  • Good written communication skills
  • Ability to speak dialects is an advantage
  • Familiarity with HMNT, MFEC, Medifund is an advantage
