Manager, System Integration (Healthier SG / Primary Care Transformation)

The role will support the planning, design and implementation of Population Health initiatives across Healthier SG and Primary Care Transformation. The role focuses on translating policy intent into implementation plans, coordinating cross-functional stakeholders, driving policy-operations-technology integration, and developing pilots and scalable solutions to strengthen primary care delivery and resident outcomes.

Key Responsibilities 

• Translate Healthier SG and Primary Care policies into implementation plans, programme roadmaps and actionable workstreams to support Population Health priorities.
• Drive programme planning and management, including governance, stakeholder coordination, progress tracking, risk management and implementation readiness across multiple initiatives.
• Lead cross-functional integration by coordinating policy, operations, technology and communications stakeholders to support successful implementation of strategic initiatives.
• Design and manage pilots, service improvement initiatives and proof-of-concepts, and develop recommendations for scaling and policy refinement.
• Support digital enablement and systems integration initiatives, including business requirements gathering, workflow redesign, testing and implementation of Population Health systems and platforms.
• Engage and manage stakeholders across MOH, healthcare clusters, primary care providers, Synapxe and internal teams to build alignment and drive desired outcomes.
• Analyse data and synthesise insights to support strategic planning, decision-making, programme evaluation and continuous improvement

Job Requirements

• Bachelor's Degree in any discipline; background in public policy, healthcare, project management, or related fields will be an advantage.
• 3 to 5 years of relevant experience in programme management, policy implementation, strategic planning, healthcare transformation or stakeholder engagement.
• Strong analytical, problem-solving and communication skills, with the ability to translate strategies into actionable plans.
• Strong stakeholder management skills, with experience coordinating cross-functional teams and driving outcomes through collaboration.
• Ability to manage multiple priorities in a fast-paced environment while maintaining attention to detail.
• Self-motivated, adaptable and resourceful, with a keen interest in healthcare transformation and Population Health.
